A car needs to be inspected either when it is first registered in the state, or it is sold to another party in Maryland. The safety inspection is very thorough, and is only required once, not yearly or every two years as in some states. The inspection takes about an hour.

If the vehicle passes, a certificate is issued and you have 90 days to register the vehicle with the MVA. If the vehicle does not pass, you have 30 days or 1000 miles to correct any failures or the inspection must be repeated.
